Understanding your FLHA · Lesson 2 of 11
What does an FLHA look like?
An FLHA, or Field Level Hazard Assessment, is usually a short form, on paper or on a phone. The crew fills it in together before work starts.
The job we will follow
The rest of this series follows this crew and this job.
Your role: General labourer.
Your crew: You, Maria, and Ali. Your supervisor plans the crew's work.
This morning: Move boxes from the loading area at the back of the building into room 3, using a cart.
This afternoon: Load a truck to send back some rented ladders, using a forklift.
Some work also needs a permit, ticket, inspection or plan. The form asks about that too.
Each part has its own lesson. The last two lessons are about what the crew does after the form is signed.
Forms look different from site to site. The names of the parts can change too, but most forms ask these same questions.
On your next FLHA
Try finding these parts on your site's form. If a part has a different name, your supervisor can explain it.
This is general information. It does not replace employer procedures, site rules, or regulations.
