Understanding your FLHA · Lesson 4 of 11

Why do we fill in a new FLHA every day?

Because today is not yesterday. The work area can change overnight. A new FLHA, or Field Level Hazard Assessment, asks the crew to look at the work as it is today.

What can change from one day to the next

  • Something spills or leaks.
  • Another crew starts working nearby.
  • Materials are delivered or moved.
  • The weather changes.

Some of these you only see by going to look. On the form, this part is often called site conditions.

Before writing, Maria walks down to room 3 to look. Yesterday the floor was dry. Today there is water on it. On the way, she sees other workers walking through the hallway.

Two ways to fill in site conditions

The task: Moving boxes from the loading area into room 3 using a cart

Not enough

Same as yesterday

Why: Yesterday, room 3 was dry. "Same as yesterday" doesn't tell the crew about the water.

Better

Water on the floor in room 3. It was dry yesterday. Other workers are walking through the hallway.

Why: Maria looked at the work area this morning. Now everyone knows what changed before they start.

Site conditions are about the area. The next part of the form is about the work itself.

On your next FLHA

Before you fill in the form, try going to look at where you will work, and ask: "What is different from yesterday?"

This is general information. It does not replace employer procedures, site rules, or regulations.
