Understanding your FLHA · Lesson 6 of 11
How do I find the hazards on an FLHA?
On an FLHA, or Field Level Hazard Assessment, a hazard is something that could hurt someone. One way to find them is to go through the task one step at a time, and at each step ask what could hurt you or someone nearby.
FLHA · Their form so far
Two ways to write the hazards
The task: Moving boxes from the loading area into room 3 using a cart
Slips, trips, falls
Why: These words could fit any job, on any day. They don't tell the crew where to look.
Step by step
- Loading the cart
- Heavy boxes could hurt your back. The damaged box could break open and fall on your feet.
- Pushing the cart down the hallway
- The cart could hit someone walking by.
- Unloading in room 3
- Someone could slip on the water.
Why: Now the crew knows each hazard, and at which step.
If you notice a hazard and aren't sure it belongs on the form, it still helps to say it.
On your next FLHA
Try picking one step of today's task and asking: "What hazard is here? What could hurt me or someone nearby?"
This is general information. It does not replace employer procedures, site rules, or regulations.
